Beginning in 1993, the proportion of the total population occupied by the elderly exceeded 7 %, which already made it an aging society. In 2018 this proportion already exceeded 14%, making it an aged society. Dementia, which usually accompanies aging, is primarily the decline of cognitive functions and the faculties of memory. Beginning with the patient’s short term memory, their orientation of three areas (time, place, people) will gradually decline. This happens according to the patient's condition and occurs in that order. Currently, the regression of cognitive functions in patients with dementia is unable to be improved with medicine. Amongst non-medicine based therapies for preventing cognition decline, the most widely used is reality orientation therapy. In recent years, the effectiveness of therapy has increased due to the use of reality orientation therapy as primary in coordination with other complementary therapeutic methods. This research has three goals: To analyze orientation therapy within care institutions and the reasons for the service deficiency through observations of current conditions; to improve the effectiveness of orientation within cognitive therapies; to introduce a product design for improving the service deficiency as well as carry out post-testing and recommendations. This research will take the observation of institutions’ reality orientation activities as primary. Integrating notes on the target subjects, results of contextual insight, behavior modeling, and through revisions implemented on early-stage experiments, we propose the following points: (1) Increased content reminders; (2) Difficulty levels; (3) Increased nostalgic elements; (4) Tabletop operations; (5) Increased interaction; (6) Decreased activity implementation thresholds. Through design guidelines, we summarize the product’s design structure and target, as well as carry out product conception and production. Last, we propose a reality orientation activity board game design, implement product verification and discussion, taking the results to be a turning point for advancements in follow-up orientation cognition product design.
