Interruptions during handover may compromise continuity of care and patient safety. Information tends to be omitted however, relating to patients’ needs and their care plan, all elements that might benefit from involvement of the patient. Objective: Handover is the best way to communicate the information of patients to the correct surgical team. The aim of the study is to exam the implementation of a checklist for preoperative handover prior to surgery and to discover the related factors of handover completeness. Methods: An action study was conducted to answer research questions from November 2018 to April 2019. One hundred and forty-three patients were randomly selected before surgery. The handover process of the nurses from the ward, pre-operating room and surgery room were observed through implementation of a checklist and a semi-structured questionnaire. Results: Information on Nursing and delivery: The Average age of the nurse in our study groups were: 27.7 5.2 (23 – 48 years old), 55 (91.6%) female. Most of them are unmarried nurses, account for 47 (78.3%) and having a family 13 (21.7%). The highest professional level is 12(54.5%) for College nursing, 10 (45.5%) for Bachelor nursing. The professional experience time was 5 years ± SD 4.1 years. Information of pre-operative nursing: The average age is 27.9 ± 2.3 (25 - 31 years old), The highest professional level is College accounting for a high rate of 6 (85.7%) and the lowest is Bachelor accounting for 1 (14.3%). The highest professional experience is 6 years and the lowest is 1 year. Single family status is 5 (%) and married is 2 (%). The related demographic factors of handover completeness: We found no statistically significance between demographic (age, gender, department, education, experience) and handover completeness, due to the study design and the less sample size. There were no reply to the semi-structure questionnaire from the nurses, but still we found some evidence to promote the quality while implementation of a checklist for preoperative handover prior to surgery. Conclusion: It is important to improve communication in handover and transferring patients by using the tool of the checklist which is standardized in handover process. Through this action study, it provided the evidence to improve the handover process. And the responsibility and communication skills are also improved.
