The Interaction of Using Citalopram and Omeprazole in Sudden Cardiac Arrest Risk Study

碩士 === 國防醫學院 === 公共衛生學研究所 === 106 === Citalopram is often used to treat depressive disorders, has recently identified the risk of causing sudden cardiac arrest (SCA). Citalopram is known to be metabolize by a proton pump inhibitor(PPI)-omeprazole. This retrospective cohort study researched the risk of SCA with citalopram and drug interaction through the 2000-2013 from LHID2000. The study found that the adjust hazard ratio(aHR) of SCA in the citalopram exposure was 2.47(95% CI 1.95-3.11) after adjusting for sex, age and the comorbidities. We also found out the aHR of SCA in the citalopram plus omeprazole exposure was 2.65(95% CI 1.46-4.80). This study also explored dose-dependent effects on citalopram by defined daily dose (DDD). The study found that the association of citalopram with SCA was affected by dose-dependent effects (cDDD=1-28 aHR=0.85(95%CI 0.54-1.33), cDDD=29-90 aHR=3.60(95%CI 2.36-5.50), cDDD=91-365 aHR=37.56(95% CI 25.25-55.90), cDDD>365 aHR=83.82(95% CI 43.37-161.00)). The results of this study expects to monitor ECG and make more relevant considerations when prescribing citalopram.
