Government promoted the “Establishment of a community care center implementation plan” to encourage non-governmental organizations to set up community care centers to provide local residents for local services. As of April 2018, there were 2,799 community care centers in Taiwan. The issue of community care is imminent, and “aging in place” has led to increase the demand on community care centers. Therefore, it is important to improve the performance of community care centers and effectively provide primary preventive care services at the local place. However, some community care centers are closed because of bad management. In order to sustainably operate the community care centers, the manager should have a model for management. Therefore, the aim of this research is to explore the non-profit organization successfully revitalizing community care center – a case study on Shan-Hu Care Association. This research adopts the case study method with qualitative research. The research subject as the representative case is Shan-Hu Care Association, through gathered data of interviews and use grounded theory to analyze the operating model of this individual study. There are three major findings. First, the dilemma is to the environment changing, searching for volunteers, and set up a community care center. Second, the key to break through the difficulties is to build a center as a “home,” and provide a center for lifelong learning. Third, non-profit organization and community developing a model to operate together is to increase the number of professionals and volunteers, who can take care the elderly and build a better caring system for the elderly. From the process, Shan-Hu Care Center has affected the diversified development in the community care center, and promote the connection between elderly and society, and allowed the non-profit organization and the community to serve together to create a “the elderly people will be properly cared for” center and successful “aging in place”. This study is found a theory, “three heart, two care, one respect.” This study is for people who are willing to serve in a community care center for references and learning.
