As high frequency of use and frequent contact with people nearby, the safety protection measures and training of forklift-operator become more important. The purpose of this study was to collect and analyze the occupational incidents of the forklifts, and to investigate the curriculum arrangements and learning effectiveness of the forklift-operator training class. The results could provide a reference for the prevention of occupational disaster related to the forklifts. This study analyzed the causes of the forklift incidents within four years (2013-2016). The students of the forklift-operator training class (with a load more than one ton) in an occupational safety and health training association of the central part of Taiwan were selected to participate in this study. The satisfaction of the course arrangement and the learning effectiveness of the subjects were investigated. One hundred and twenty questionnaires were valid. The Wilcoxon signed rank test was used to compare the difference in cognitive level before and after the forklift training. The survey showed that after the course learning, the average cognitive level of the “driving device structure”, “loading and unloading device structure”, “operational mechanics” and “safety protection” was significantly improved (p<0.001). The average satisfaction after the technical course exceeded the “satisfied” level. The average satisfaction level of the subjects with the training association also exceeded the “satisfied” level. According to the results of the analysis of occupational incidents for forklifts, mostly the type of the incidents was collision (43%). Most of the incidents were caused by the failure to implement special or general safety and health education training for most forklift operators (98%). It highlighted the importance of the forklift training. Although the content of the academic or technical curriculum were in saftisfied condition, the arrangement of the driving practice venue was very difficult. It is recommended that computer simulations of forklift driving including the situations which occupational incidents occurred frequently (collision) can be used to enhance the learning interest and strengthen the awareness of safety driving. As for the disaster prevention of forklifts, the safety and health on-the-job training, job risk identification, automatic inspection, well management of driving line and constant supervision of the safety behavior of the forklift operators.
