The increasing number of physically and mentally disable population was noted due to multiple chronic conditions with different medical landscape. In such rapid changes in an ageing society, the current pattern of medical care service would undergo a great modification, which is implying the tremendous need in terms of flexible and integrative medical care. Bureau of National Health Insurance has carried out the ' Integrated Home Health Care Plan ' since year 2016 to conform some pilot projects, such as home nursing care reimbursed by national health insurance, integrated delivery system of long-term mechanical ventilation, the present and future of community/home-based palliative care and the patients receiving home medical care combined with four services, for the three care stage. That encourage related-institutions constitute the team about integrated-care connecting patients to rehabilitation service and other related resources. Under the team, each plan must horizontally and vertically connect to the other so that the program can work effectively. The integrated home health care has been spotlighted recently by the developed countries due to reducing unnecessary admission, diminishing the medical expenditure, and facilitating user-friendly care service in communities. The purpose of this study is to investigate the members participated in the Integrated Home Health Care Plan in a regional hospital located in Taichung. A total of 75 questionnaire were returned and 63 were valid which yield a valid response rate of 84%. The questionnaire items include their paritication, their perceived performance about, and their attitudes toward the Integrated Home Health Care Plan. The data was analysed according to the following procedure: (1) The questionnaire used in this study was adapted from the previous researches about performance evaluateion of medical plans. (2)Data were analysed by factor analysis, one-way ANOVA and multiple regression analysis. As a result, we had the following conclusions. The results indicated that Organizational Factors, Administrative resource management, Human Resource Management and Professional capability have a significantly positive impact on the attitude of participants of Integrated Home Health Care Plan. However, the only critical factor affecting the perceived effectiveness of the Integrated Home Health Care Plan is Professional Capability. That is, only well trained and prepared plan professional team can improve the plan performance. In general, due to the early stages of the project, the effectiveness of the plan is still limited.
