As a result, Taiwan has entered an aging society. This study took Zhongtai Vil in Xinzhuang District of New Taipei City as an example to conduct an in-depth study and investigation on the relationship between community health management for the elderly and participation in leisure activities. This paper used questionnaires and the questionnaires are filled in by the elders or by the head of a basic community unit in person after interviewing. This study handed out 255 questionnaires and recovered 254 copies. The recovery rate was 98%. Respondents were mostly aged 56-65, accounting for 40% of all (40.9%). More than 60% of the respondents had already lived here for over 15 years (66.9%), and nearly 70% of the respondents did a health check with health insurance (177 people). 46.9% of the respondents were examined and confirmed the disease by a doctor, and 71 of them were hypertension (22.9%). There were 117 respondents participating in leisure activities due to family, accounting for 46.1%. The respondents participated in leisure activities for the following reasons: the most people participated because of their interests, 88 people in total (34.6%), followed by for their health, 60 people in total (23.6%), and for relaxation (13%). In the overall aspect of disease and leisure participation, there was no significant difference in diseases such as "arthritis," "kidney," "other diseases," and "multiple diseases", but there were significant differences in "diabetes" (P=0.023), "cardiovascular" (P=0.003), "high blood pressure" (P=0.026), and "no disease."
