碩士 === 長庚科技大學 === 健康照護研究所 === 106 === Abstract
Background: A review of the literature review was conducted; most scholars' literature was based on job burnout among emergency room nurses, work team communication, job satisfaction, technical environment maintenance, occupational hazards, etc., but little discussion was conducted on the work stress of nurses in the emergency room. Heart rate variability and its related factors.
Objective: The main purpose of this study is to explore the relationship between work stress and heart rate variability related factors in nurses in the emergency room, and to reduce the work pressure of emergency nursing staff. It is expected that the results of the study will provide a reference for the management of personnel in nursing administration.
METHODS: Using the cross-sectional study method, all nursing staff who had an emergency nursing staff at a medical center for more than three months were selected as the study object. The questionnaire was measured using the “Nursing Staff Work Stress Scale” developed by Cai and Chen (2000). Work stress, including four aspects of personal response, job attention, competence, and inability to complete private work, and measurement of heart rate variability parameters (SDNN, LF, HF, LFa/HFa, PSI, MSI) for correlation analysis and description Statistical analysis of sexuality and inference and comparison of relatedness.
Results: There are significant differences (1).work stress, and HRV scores; (2) Work pressure and two aspects of “competency and inability to complete private tasks” in the personal attributes section and “job competency”. The average monthly salary is more than 50,000, and the highest score is the weekly exercise minutes of 60 to 120 minutes. Personal attributes and "unable to complete private affairs": to live with children, 31-40 years of age, the total length of care of more than 6 years, the critical years of serious illness more than 6 years, advanced rank higher than N3. The “LF” in heart rate variability was highest in men, with exercise habits, and more than 3 times weekly exercise. The "MSI" of heart rate variability is female, 21 to 30 years old, unmarried, childless, certified by an acute and severe nursing practitioner, no exercise habits. The total number of years of nursing is less than five years, and the critical age is less than five years. Less than 50,000 salary, advanced ranks with the highest score of N0-N1, there are significant differences between groups afterwards. (3) Correlation between work stress and heart rate variability: There is a significant difference in negative correlation between heart rate variability LF/HF, indicating that the higher the LF/HF is in response to work stress responses. The more active the sympathetic nerves, the higher the work pressure, while the work-related aspect is negatively related to the PSI. The more the body fatigue is concerned, the less able to focus on the work situation. The negative correlation between PSI and MSI indicates that the physical and psychological pressure is too great. No effort to complete his private affairs. (4) The predictive factor for "competency at work" is an average monthly salary of 50,000 yuan or more; the predictive factor for "unable to complete private affairs" is the total number of years of nursing seniority, and the predictor of "LF" in the heart rate variability parameter. For males; the predictor of "MSI" is N2 Advanced Rank.
Conclusion/Practical application: Based on the above conclusions, reducing the work environment for emergency nurses to work under pressure is an important issue in the nursing field. It is also a problem of patient safety and is an important economic issue for hospital operators. The premise of reducing work pressure is to create a good organizational relationship, complete equipment, and a humane work environment. This study provides hospital managers and nursing administrators with a direction to reduce the work pressure of emergency nurses.
