However, other countries study revealed children having unilateral cochlear implant without bilateral hearing advantage are still facing some difficulties, especially in speech perception in noise and bilateral cochlear implant can improve these difficulties. However, there is still lack of bilateral sequential cochlear implant outcome study in Taiwan Mandarin speaking user. The study purpose is to probe speech perception outcome in our Taiwan Mandarin speaking bilateral sequential cochlear implant recipients and further study the influence of inter-implant interval on these results. Methods: There were 8 Taiwan Mandarin speaking bilateral sequential CI subjects including 5 school age children, 1 pre-school children, and 2 adults. Seven subjects had speech perception objective outcome measure and 1 had PEACH subjective outcome survey. Four school and 1 adult CI recipients received speech outcome measure after 2nd CI 6 months and 12 months sequentially and 1 school and 1 adult after 2nd CI in 22, 32 months. The speech perception outcome objective tests included Mandarin Monosyllable Recognition Test(MMRT), Mandarin Hearing In Noise Test-Quiet (MHINT-Q) and Mandarin Hearing In Noise Test-Noise 5 dB SNR (MHINT-N) and subjective questionnaire outcome survey were also done after 2nd CI 12 months. Results: MMRT, MHINT-Q and MHINT-N results revealed after 2nd CI 12 months is better than 6 months, although no significant difference. There was a negative outcome correlation trend between inter-implant interval and 2nd CI hearing and binaural CI hearing. Bilateral CI was better than 1st CI or 2nd CI in after 2nd CI 6 months and 12 months, although no significant difference. Eight subjects all attained positive attitude on their 2nd CI in their subjective questionnaire survey. Conclusion: Speech perception outcome can get better after longer CI use duration in 2nd CI and bilateral CI and also better for those with shorter inter-implantation interval in the 2nd CI and bilateral CI. Bilateral CI hearing is better than 1st or 2nd CI hearing in the MHINT-Q and MHINT-N objective tests, although these showed no significance. All 8 bilateral sequential CI recipients attained positive attitude, worthy thought and would suggest others unilateral CI users to get 2nd CI by their subjective questionnaire survey.
