In order to meet the increasing long-term care (LTC) needs, advanced countries have begun to study the service of LTC. In order to make good handling of LTC resources, some countries have established a well organized LTC system, a computer-based “system of care-needs certification” was designed to categorize various care needs level for elderly based on actual time spent by staff members on providing care. Nowadays, Taiwan has no actual LTC content, specific item or operational definition, rather, to deliver the care services by “time” as a practical indicator in the care area. Purpose: To understand the daily ADLs care services in LTC facility (nursing home), the possible operating time, the related grossly percentage, frequency and distribution. Methods: To use “one-minute time-and-motion study” as a method to investigate the amount of care on ADLs provided by custodial care staff in a LTC facility (nursing home) in Taiwan. It’s followed on a total of 50 residents and conducted more than 24 hours. Result: In terms of care frequency and the sum of time, "transfer / movement care", "eating (intake) care" and "excretion care" are the main stream of ADLs care; the average of operating time of ADLs care results as follows, the "bathing care" is the longest, followed by "eating care" and "dressing care". Most of the ADLs care activities take place during the day time (06:00 ~ 18:00), which be taken about 1.5 to 2.5 times more than those in night time (except for the dressing care), and the minimum of care occurred at midnight (esp. 00:00 ~ 04 : 00), all day ADLs care will gradually re-start during 04:00 ~ 06:00. Conclusion: Among ADLs, "transfer / movement care", "eating care" and "excretion care" be regarded as the main stream. The longest operation is “bathing care”, i.e. more manpower resources mandatory. In case of quantitative approaches for the LTC service, the observation based on "time" can be embodied in the practical operation. And the "one-minute time-and-motion method" can be well organized fully compliant with the care operation. This study has provided a preliminary experiences of LTC operating, which can be used as a reference or inferential basis for resource allocation. Heretoday in Taiwan, the LTC policy has been trapped for long years to be delivered as the non-professional or general affairs for execution. Along with the old operation framework, the related LTC needs, supply, distribution, and fluctuation be still uncertain, never be assured all the way, and so it’s impossible to allocate the limited national resources under rational basis. Recommendation: This study implies that the public sector should focus on and leads the case; with quantitative operation approaches to LTCs, try to organize the related study in large scale, and to validate the related definition, operation, and implementation in the field, let LTC be settled up from the darkness to dawn.
