Summary: | 碩士 === 國立中興大學 === 獸醫學系暨研究所 === 105 === Spontaneous chronic corneal epithelial defects are superficial ulcers that fail to resolve through normal epithelial wound healing, and no established treatments have been described. The conventional medical therapeutic options of corneal epithelial defect are intensive artificial tears, topical antibiotic, and corneal debridement. In recent studies, autologous serum drops have been effective in improving and stabilizing signs and symptoms in patients who do not improve with conventional therapy in nonhealing corneal ulcer. However, autologous serum may be unavailable in some patients. Thus, we hypothesized allogeneic serum eye drops would be an alternative treatment for the corneal ulcers which have had no improved with conventional methods. The purpose of this study was to evaluate the efficacy and complications of allogeneic serum eye drops on spontaneous chronic corneal epithelial defects in dogs.
Twelve eyes of 6 clinically healthy dogs have undergone a preclinical test. According to the results of the modified Draize test, there were no irritations of the eyes. As for the clinical trial, there were fifteen eyes of 12 patients with spontaneous chronic corneal epithelial defects who had not recovered after 2 weeks of conventional treatment were included. All patients were treated with allogeneic serum eye drops, topical antibiotics, oral systemic tetracycline, and had been undergone diamond burr debridement. The corneal wounds were photographed and measured by ImageJ image analyzing software. Standardized forms and Modified MacDonald-Shadduck Scoring System were done by attending ophthalmologists. Five eyes healed within 1 weeks, five eyes healed within 2 weeks, and two eyes healed within 1 month. The average healing time was 12.25±5.28 days. Three eyes failed to heal within 1 month and altered to use other treatment. The average follow-up period of the recovery patients was 69.56±59.05 days. During the follow-up period, there was no recur and no significant side effects were detected. In summary, the results of this study indicated that allogeneic serum eye drops is an effective and safe treatment for spontaneous chronic corneal epithelial defects in dogs.
