Removable dentures commonly were fabricated for patients with numerous missing teeth. There were many problems with denture retention or stability when treating lower edentulous patient, especially poor oral condition. Dental implants were used to assist denture for better tissue support for many years, but they were more expansive and treating time. No systematic reviews about comparing implant supported overdenture and conventional denture on quality of life, especially patient-centric outcomes. Hence, meta-analysis was conducted.
Research methods:
A literature research between January 1996 and December 2016 were performed using strict eligibility criteria including OHIP as measurement tool on PubMed, MEDLINE, Cochrane Library and Google scholar. The reviewers screened titles, abstracts and performed full-text screening of studies. A random effects model was applied to measure the mean difference in OHIP scale on CMA ver. 3.
Results:
Twelve studies were reviewed and included. Results of 1004 patients (531 implant-supported and 523 conventional) were pooled for analysis of outcomes. Results of analysis with a standard difference in means (95% confidence interval) in the score of -0.598 (-0.728 , -0.468) for total scores of OHIP; -0.666(-0.921 , -0.41) for functional limitation; -0.81(-0.948 , -0.672) for physical pain; -0.611(-0.833 , -0.389) for psychological discomfort; -0.703(-0.916 , -0.491) for physical disability; -0.404(-0.636 , -0.173) for psychological disability; -0.326(-0.515 , -0.152) for social disability and -0.353(-0.511 , -0.155) for handicap. All dimension revealed statistically significant difference favoring the implant group on oral health related quality of life.
Conclusions and recommendations:
Our findings suggest that the implant-supported overdenture performed better in regard to quality of life as measured using the OHIP questionnaire. But there are still some factor affecting the treatment outcome. Further studies are required for more evidence on the two treatment options.
