Materials & Methods: This retrospective study consist of 37 consecutive adult Taiwanese patients with skeletal Class III malocclusion and mandibular deviation more than 4 mm. The patients were divided into 2DP and 3DS groups. Preoperative and postoperative 3D cephalometric measurements using cone-beam computed tomography were obtained for all patients. The outcome variables were linear and angular measurement difference in the preoperative and postoperative between the 2 groups. Results: A total of 4 cephalometric variables, namely the bilateral gonion on the nondeviated side to the midsagittal plane (MSP), midgonion to the MSP, upper first molar on the nondeviated side to the Frankfurt horizontal plane (FHP), and yaw angle, were statistically significant between the 2 groups. In 3DS group, mandibular symmetry was achieved because the center between the bilateral gonion was improved, and the horizontal gonion (Go to the MSP) between the deviated and nondeviated sides after the surgery showed no significant differences. Conclusion: 3DS provides all the necessary information of planned surgical movements for facial asymmetry correction. The surgical outcomes were improved and mandibular symmetry was better achieved with 3D planning. The present results suggest that 3DS should be considered during surgical planning to improve surgical outcomes, particularly the achievement of bilateral mandibular contour symmetry. W. C.
