Summary:碩士 === 國立雲林科技大學 === 資訊管理系 === 104 === With the rapid progress of the Internet, coupled with the increasing health consciousness of the people, the Ministry of Health and Welfare health passbook system introduced in Taiwan in 2014. People can use the computer or mobile phone easy access to personal medical information, and physicians can take advantage of the system to provide better service. However, the information system is a social system of science and technology, in addition to investment and to improve the information technology factors influence acceptance of intent is an important research topic. This study combined theory of planned behavior and health belief model established research framework for verification. A total of 422 questionnaires were collected through the Internet valid samples, and structural equation modeling analysis and measurement mode configuration mode. The main conclusions can be aggregated as follows: (1) perception of trust, perceived risk, and perceived benefit significantly positive impact on the attitude of the health book, (2) the attitude and subjective norm significantly positive effect on access to health passbooks of intent, (3) perceived behavioral control the use of the intended impact on the health of the passbook as expected. Research conclusions can provide effective health promotion an important reference book.
