碩士 === 東吳大學 === 財務工程與精算數學系 === 104 === In this study, we discuss risk factors for secondary cancers for top ten cancer patients in Taiwan based on data from the National Health Insurance Research database during 1997 to 2013. Definition of the top ten cancer patients with ICD-9-CM code suffering of one of the top ten cancers and have catastrophic illness card information. A sample of 715,125 cancer patients is observed from 1997 to 2013. The method we use estimates the cumulative incidence rate of patients by the Kaplan-Meier estimator, and subsequently uses the log-rank test to identify possible risk factors. Finally, we model the secondary cancer hazard function by the Cox proportional hazards model. As such, we calculate the cumulative incidence rates by Cox proportional hazards model, and then use it to calculate a ten-year insurance premium, benefits including the occurrence of a secondary cancer type. The study results show that gender, cancer site, and age are risk factor for secondary tumors. Consequently, we recommend a net single premium for patients, over 60 with esophageal cancer patients. These patients should pay the highest premiums. However, males between 0 to 39 with thyroid cancer and females between 0 to 39 with breast cancer should pay the lowest premiums.
