Summary: | 碩士 === 國立臺北護理健康大學 === 健康事業管理研究所 === 104 === Objectives: Exploring the incidence and the factors associated with
hypercalcemia in hospitalized patients receiving total parenteral
nutrition (TPN) therapy.
Methods: This was a retrospective cohort study investigating 350 patients who
had received TPN therapy at a medical center in northern Taiwan during
the period from January 1, 2013 to June 30, 2014. The data were
extracted from each patient’s hospital chart, which included: basic
characteristics, the nutritional status before TPN therapy, and the TPN
therapy parameters. Multiple logistic regression analysis was performed
to investigate various factors associated with the incidence of
hypercalcemia in these patients.
Results: In this study, the incidence of hypercalcemia was 8.6% in hospitalized
patients who received TPN therapy. Independent factors which
significantly affect the incidence of hypercalcemia in these patients
included: hemodialysis, body mass index, and the duration of TPN
therapy. Patients who received hemodialysis had higher probability to
develop hypercalcemia compared to those who had not received
hemodialysis (AOR = 23.752). Patients with body mass index (BMI) <18.5
kg/m2 had higher probability to develop hypercalcemia compared to those
with BMI ≧18.5 kg/m2 (AOR = 3.724). Finally, patients who had received
TPN therapy for more than 14 days were more likely to develop
hypercalcemia compared to those with TPN therapy less than and equal to
14 days (AOR = 5.013).
Conclusions: Hemodialysis, BMI, and the duration of TPN therapy were important
factors associated with the incidence of hypercalcemia in
hospitalized patients who received TPN therapy.
