People with disabilities demand more health care needs and special education for their social adaption. According to statistics of the Ministry of Education from 2004 to 2014, number of high school students with disabilities increased 8706 persons and up to 23529 students with disabilities accounting for 3.1% in whole high school students. In this study, we aimed to investigate the transition status and the relating associated factors among high school students with disabilities graduated from senior high schools. Method This is a retrospective cohort study, samples come from the releasing database of Special Needs Education Longitudinal Study. Totally 830 persons with completed survey data following at three stages of 10th grade, 12th grade and one-year after high-school graduation were included. Personal, familial and school related data were collected for prediction of later transition status after senior high-school. Result Totally there are 830 samples included in this, and were categorized into three groups according to their transition status as with successful transition to university, workplace or none of them. There are 534 (64.3%) and 160 (19.3%) graduates with successful transition to university or workplace respectively, and 136 cases (16.4%) without transition to university or workplace. Analyzed data showed that good living skills, placement in resource classroom were helpful for later successful transition. Conclusion In this study, results showed that developing living and decision-making skills at high-school stages are important for students with disabilities for later successful transition after leaving school.
