Without proper software hazard assessment process, business continuous development may be interrupted and job losses may occur. MIL-STD-882E was developed by the Department of Defense of USA to demonstrate how to track software risk assessment information. In this research, software of hazard tracking system is developed based on the principles of MIL-STD-882E. The FMEA method is applied to some industrial cases to illustrate how to construct an acceptable formal risk assessment operation model which can get actual reality of how to track software hazard of track system.
