The individual emotional reactions are driven by applying sonic resonance physical phenomenon, in order to relax the muscles. Music can raise awareness, improve sleep quality, reducing anxiety to calming effect and help sleep; sleep and depression are common symptoms in patients with chronic obstructive pulmonary disease. However, the use of music therapy is uncommon in patients with chronic obstructive pulmonary disease (COPD). The purpose of this study was to explore the effectiveness of music therapy on improving sleep and depression of patients with COPD. Using quasi-experimental study design, 40 COPD patients were block assigned in a random manne, into either experimental group or control group. There were 22 and 23 in-hospital patients in the experimental and control group, respectively. The patients in the experimental group listened to a 45-minute music before bedtime for 5 consecutive days, while the control group was under usual care. Three repeated measures were day one night before the music therapy (1st measure), day three morning? (2nd measure), and day six morning (3rd measure). Patients completed the Verran & Snyder-Halpern sleep scale and Taiwanese depression questionnaire. Preceding and proceeding each music therapy session, heart rate, respiration rate, and blood oxygen concentration were recorded. Using the SPSS18.0 software, descriptive and inferential statistics analyses were performed. Prior the intervention, heterogeneity between the experiemental and control groups was statistically insignificant. The results found that, after five days of music therapy intervention, depression in the experimental group decreased from 31.68±8.81 to 19.27±6.34 (t=8.621, p=.00), while the control group increased from 28.78±10.40 to 30.35±12. 96 (t=-1.314, p=.89). There was significant improvement in depression in the experimental group compared to the control group (F=42.803, p=.00). In sleep quality, the experimental group increased from 546.45 ± 131.48 to 811.82 ± 174.87(t=-8.819, p=.01), while the control group decreased from 633.35 ± 190.72 to 557.04 ± 155.81 (t=2.676, p=.33). There was significant improvement in sleep quality in the experimental group compared to the control group (F=45.935, p=.00). There were no significant differences between the experimental group and the control group in physiological indicators, including heart rates (F =0.062-1.932, p=0.804-0.172), respiratory rate (F= 0.458-0.054, p=0.502-0.818), oxygen saturation (F= 0.008-0.658, p=0.928-0.422). Overall, music therapy can improve sleep quality of patients with COPD, and reduce their depression. It is recommended that clinical nurses try to implement music therapy as an routinepractice approach in caring patients with sleep difficulty or depression. Future studies are needed to test effectiveness of music therapy in community-dwelling adults to enhance the professional care at nursing practice.
