The data input by the user can be obtained with preset form. The form is a template generated by designer in nature. A part of the template is the hint texts provided by the designer, while the rest is the plug-ins used to get the data input by user, such as textbox and drop-down menu. If a flexible and easy-to-use form generator is provided, it will be very easy for the form manager to get the needed form and analyze the data input by the user with some tool. However, the popular form generators, such as MS Word and Google Docs can only generate the forms in the format like questionnaire survey. The traditional form can’t provide appropriate data input format for the field that requires template with the plug-in embedded in the form property, such as the medical case input on the medical information system. In this study, we develop a web-based form generator that can be provided for the manager to design plug-in embedded in the form property. Moreover, it can convert the form filled by the user into Word format, so the manager can output the paper file.   The graph structure can be applied in a lot of fields. For example, the Resource Description Framework (RDF) used to describe the correlation the network resources in the semantic network and record the relationship between the patients and their families in the medical information system can be represented in a graph structure. The structure of a traditional graph focuses on the correlation between vertexes, but rarely discusses the content of vertex and the correlation between the points. Different from the traditional graph structure, the heterogeneous graph permits the vertexes in the graph can have different structures (properties), and the sides of the connection points in the same graph can have different IDs (names), which is superior to the traditional graph structure in terms of the scalability and portability. It can be used to describe the network formed by various correlations between different entities. Therefore, it can provide more flexible support for describing RDF, family tree and other graph structures. However, the query of graph structure is limited to the traditional search method, such as breadth-first traversal search, which derives quite high cost in computation. In this study, we provide an interface for the user to draw a heterogeneous graph, including the vertexes of different structures and sides with different IDs. After it is completed, the system will analyze the graph drawn by the user, convert it into the relational structure and save it in the relational database. When querying the data in the graph, the user may use the standard database query language SQL, so as to avoid the computation cost taken by the traditional search method. Andy S.
