Irritable bowel syndrome is one of the most frequently encountered illnesses in outpatients of the female population. We investigated the hypothesis that endometriosis may increase the risk of irritable bowel syndrome in Taiwan population by using a population-based data set. Our study cohort consisted of women diagnosed with endometriosis between January 2000 and December 2005 (n= 6,143). For each endometriosis patient, five age-matched control women were randomly selected as the comparison cohort (n= 30,715). All participant cases were followed for 5 years from the index date to identify the development of irritable bowel syndrome. Cox proportional-hazards regression was performed to evaluate the 5- year irritable bowel syndrome- free survival rates. The results show that 894 women developed irritable bowel syndrome during the 5- year follow-up period, among whom 252 were endometriosis patients and 642 were in the comparison cohort. The adjusted hazard ratio (HR) of irritable bowel syndrome in patients with endometriosis was higher [HR, 1.94; 95% confidence interval (CI) = 1.67- 2.24; p< 0.001] than that of the controls during the 5- year follow-up. We found increases in the risk of irritable bowel syndrome in women aged 20- 49 years : 20- 29 years (HR, 1.92; 95% CI= 1.33- 2.77; p< 0.001), 30- 39 years (HR, 2.17; 95% CI= 1.62- 2.79; p< 0.001) and 40- 49 years (HR, 1.80; 95% CI= 1.47- 2.22; p< 0.001).
