To accelerate the medical record availability for patients and medical staffs and collect all medical records into a database, most hospitals and medical centers introduce the electronic medical records into hospital systems. The electronic procedure is listed as the first priority in large medical institutes these years. However, the development of the electronic medical records encounter difficulties from planning stage, introducing stage to executing stage. Moreover, the achievement of the data exchange in the hospital and between different health institutes is also the key point for the government. In Yunlin County, there are two types of medical institutes: one is local community hospitals and the other one is metropolitan hospitals. The metropolitan hospitals are branch hospitals of three Academic Medical Centers located in South-Taiwan, Central-Taiwan and North-Taiwan, respectively. This research discusses the reformation progress before and after the electrical medical record. Furthermore, the influences of electrical medical records in the hospital and data exchange between health institutes are included in this research as well. The purposive sampling is used to interview nine respondents, including staffs of the medical care/affairs division, information management division and medical record management division in health institutes. The semi-structured interview of the qualitative research is employed in this research. Then, the grounded theory is chosen to analyze the interview data. The open coding and axial coding are cross applied to the interview data. Finally, the selection coding and generalization procedure are used to conclude the effect factors of the progress of electrical medical records in the three respondent health institutes. In this research, the effect factors are separated into three aspects: (1)the internal change of the institute; (2) the information skill; and (3)the data liquidity. The internal change of the institute includes the difficulty of digitalizing medical records, the difficulty of executing electronic signature, the consensus between staffs and the impact to the staff of medical record division. The information skill indicates the system integration, the data security and the privacy of personal data.
