Swaziland has been classified by the World Health Organization as a “human resource crisis” country, amongst 35 countries in Africa and 57 countries in the world. Poor working environment, lack of adequate supplies insufficient supervision, poor on the job training continue to drive health workers including midwives. Objectives: To determine the level of job satisfaction of midwives in Swaziland and their intention to quit their job. Methodology: Cross-sectional study using survey method .Self-administered questionnaire was used for data collection. Results: 96.3% (65) of the midwives are not satisfied with their workload at work, 95.5% (64) are not satisfied with working place physical environment, 87.6% (59) are not satisfied with their pay and 87.3% (59) are not satisfied with rewards in their work place. The level of job satisfaction amongst the midwives is 72.8% (49) and 48.3% (32) have an intention to quit their job. Conclusion: This study found that midwives in Swaziland are more dissatisfied with their job and physical environment, co-workers satisfaction, workload and nature of work are associated with overall job satisfaction. A recommendation to the ministry of health in Swaziland will be to provide working equipments and protective clothing in maternity departments in order to improve physical environment satisfaction which will in turn increase midwives level of job satisfaction.
