However, they are all on a national scale. There have been only very few analyses and studies of individual cities and counties. Consequently, Hsinchu County's integrated screening of the Three Highs' prevalence and its relevant factors are very worthy of studyingand practical application.
Objective: We studied the integrated screening health data of Hsinchu residents over 40 years of age, and first analyzed prevalence of hypertension, high blood sugar, and high lipid and second explored its associated factors to understand the distribution of the townships in screening population. The goal is to target the population for preven-tion and treatment to enable drafting screening for prevention and treatment strategies as well as intervention measures to prevent the diseases from happening.
Method: Based on research objective, correlational study were conducted using sec-ondary data. The data collecting duration for this research was from January 1st, 2010 to December 31st, 2013. Each included case was observed only once. It is a cross-sectional study based on the data collection time. The study subjects were Hsinchu County's residents who participated in the integrated screening service and were over 40 years of age. The statistical data sources to be analyzed were secondary data from the screening's physical examination result and blood test value filing. Studies were done based on variables of gender, age, township, BMI, waistline, blood pressure, blood sugar, total cholesterol (TC), triglycerides (TG), and high-density lipoprotein cholesterol (HDL-C) to understand the Three Highs prevalence among Hsinchu residents over 40 years of age and the related factors, using descriptive statistics (frequency, percentage, average, and standard deviation) and inferential statistics (Chi-square test, one-way analysis of variance (ANOVA), Post-hoc Scheffe, and logistic regression analysis) to analyze data.
Results: This research examined 17,133 subjects, with 10,382 women (60.6%). The average age was 63.37 ± 12.56 years of age. Of them, 6,208 (36.2%) were ≥ 70 years old and were the most numerous. The sample had average screening participation of 1.24 times, whereas 80.8% of the subjects only participated 1 time. Looking from the perspective of townships, Huko Township had the most participants with 4,811 (28.1%). The prevalence of hypertension, high blood sugar, high lipid were 35.3%, 13.5%, and 32.5% respectively. Chi-square analysis suggested age, BMI, waistline, township were correlated to hypertension, high blood sugar, and high lipid with statistical significance (p< .05). One-way ANOVA suggested gender, age, BMI, waistline, township are correlated to systolic pressure, diastolic pressure, blood sugar values, TG, and TC with statistical significance (p< .05). Logistic regression analysis suggested hypertension patients' risk of high TC is 1.59 times that of a healthy person. High blood sugar patients' risk of high TG is 2.29 times that of a healthy person.The risk of having high blood sugar in people with overly high TG is 2.35 times that of a healthy person.The risk of having overly high TC among people with overly high TG is 3.64 times that of a healthy person.The risk of having hypertensionamong people with overly high TCis 1.55 times that of a healthy person.The risk of having overly high TG among those with overly high TC is 3.43 times that of a healthy person. Patients with HDL-C abnormality have risk of having overly high TG that is 4.46 times that of a healthy person. Patients with HDL-C abnormality have lower risk of having high TC, at 0.21 times that of a healthy person.
Conclusions: The result can serve as a reference for health administrative agencies in planning and policy-making. This suggests the integrated screening should be contin-ued. However, careful planning is required to encourage people to participate to im-prove screening rate and re-examining rate. Three-High cases should be targeted to establish medical care referral and a follow-up system, connecting preventive health care and medical care to realize the follow-up management of irregular individual cases.
