Care providers through contact with contaminated ward surfaces may play an important role in transmission of HAI pathogens. Many studies described over 50% of infection incidents were preventable by team resource management (TRM) as a remedy to rectify personal and environmental factors. Objective The aims of this study were to determine if the application of TRM would improve the environmental cleaning, and to evaluate the HAI rate after the environmental cleaning intervention in RCC. Methods/Steps This study was performed at RCC with 8-bed capacity in a regional teaching hospital of Taichung. The environmental cleaning of TRM intervention consisted: (a) a change for applying disinfectant to cleaning equipment, (b) setting a guideline for housekeeping practice, (c) filming ward cleaning procedure for education, (d) using check list to audit the room cleaning procedure. By Gram’ stain method, we cultured eight commonly touched surfaces in each bed of ward immediately after housekeeping cleaning (less than 30 minutes). Results/Findings After a 24-week implementation of the TRM in RCC, we found that of the 8-bed of ward, the negative environmental cultures on the surface were largely increased from 19.09% to 76.36%. However, the mean HAI incident rate was slightly decreased from 3.38‰ (Jan-Oct, 2014) to 3.9‰ (Feb- Apr, 2015). Conclusion To conclude, the implementation of TRM had made some progress in the completeness of environmental cleaning. Besides, it reduced the spread of environmental microflora and the possible risk of HAI incident in RCC. Hsu, Che-Han 許哲瀚 2015 學位論文 ; thesis 60 zh-TW |

碩士 === 中臺科技大學 === 醫療暨健康產業管理系碩士班 === 103 === Introduction
In recent years considerable concern has arisen over the environmental surfaces contamination of healthcare-associated infection (HAI) in Respiratory Care Centers (RCC). Care providers through contact with contaminated ward surfaces may play an important role in transmission of HAI pathogens. Many studies described over 50% of infection incidents were preventable by team resource management (TRM) as a remedy to rectify personal and environmental factors.
Objective
The aims of this study were to determine if the application of TRM would improve the environmental cleaning, and to evaluate the HAI rate after the environmental cleaning intervention in RCC.
Methods/Steps
This study was performed at RCC with 8-bed capacity in a regional teaching hospital of Taichung.
Results/Findings
After a 24-week implementation of the TRM in RCC, we found that of the 8-bed of ward, the negative environmental cultures on the surface were largely increased from 19.09% to 76.36%. However, the mean HAI incident rate was slightly decreased from 3.38‰ (Jan-Oct, 2014) to 3.9‰ (Feb- Apr, 2015).
Conclusion
To conclude, the implementation of TRM had made some progress in the completeness of environmental cleaning. Besides, it reduced the spread of environmental microflora and the possible risk of HAI incident in RCC.
