Mounting evidence shows that sleep disorder not only links to poor quality of life, but also links to cardiovascular diseases and even mortality. However, the relationship between sleep disorder and cardiovascular diseases has not been examined in population level in Taiwan. We conducted a retrospective cohort study to investigate the relationship between sleep disorder and cardiovascular diseases. Data was obtained from Longitudinal Health Insurance Database (LHID) 2005, which contains original claim data of 1,000,000 beneficiaries enrolled in year 2005. They were randomly sampled from the same year registered beneficiaries of the National Health Insurance. We utilized longitudinal data from year 1999 to 2010. Case with sleep disorder was defined if their diagnoses from outpatient and inpatient claim data in year 2000 containing ICD-9-CM 307.4 or 780.5, and had no predefined cardiovascular diseases. We selected common cardiovascular diseases including hypertension, coronary artery disease, and cerebrovascular disease, defined by diagnoses from outpatient and inpatient claim data. Controls were selected from beneficiaries who had been admitted in 2000, and without the diagnosis of sleep disorder and aforementioned cardiovascular diseases. Cases and controls were matched by propensity score methods. We use Cox regression to analyze the effect of sleep disorder on cardiovascular diseases. Between year 2005 and 2010, the prevalence of sleep disorder was around 4.52% - 5.55%. In the subjects with sleep disorder, the 10-year cumulative incidences of hypertension, coronary artery disease, cerebrovascular disease, combined cardiovascular diseases were 38.98%, 23.67%, 14.65%, and 49.27%, respectively. After adjusted for sex, age, urbanization level, income, job, anxiety disorder, depressive disorder, diabetes, and comorbidity index score, sleep disorder was associated with a higher risk for hypertension (adjusted hazard ratio (aHR) 1.19, 95% confidence interval (C.I.) 1.13-1.25), coronary artery disease (aHR 1.26, 95% C.I. 1.18-1.34), cerebrovascular disease (aHR 1.13, 95% C.I. 1.04-1.23), and combined cardiovascular diseases (aHR 1.25, 95% C.I. 1.20-1.31). In this population-based, retrospective cohort study, we found that sleep disorder is significantly associated with higher risks for incident hypertension, coronary artery disease, and cerebrovascular disease.
