Clinical studies pointed out that pregnant women with mild to severe of periodontal disease have prevalence rate about in 30%. The higher prevalence of periodontal disease in pregnancy may be considered as a risk factor for preterm birth, through some studies on the impact of periodontal disease on preterm birth. Objectives: To determine the association between periodontal disease and preterm birth for women in Taiwan. Methods: This study used National Health Insurance Research Database (NHIRD) one million beneficiary dataset, therefore it is a secondary data analysis and case control study, The independent variable is the diagnosis of periodontal disease, the dependent variable is diagnosis of premature, pregnant women during with the period from January 2006 to December 2010. After a propensity score matching with basic characteristics including age, gestational hypertension, gestational diabetes in 1:4 match, select the data with similar characteristics between the case and the control groups, case group have 1,455 subjects, and controlled group sample have 5,820 ones, totally is 7,275 subjects, The confounding factors include social status, the Resource Utilization Bands (RUBs) one year prior to delivery, and disease history of subjects. Result: The periodontal disease and pregnancy outcome of prematurity are not statistically correlated. All control variables, including before preterm of history one year prior to delivery, hypertension, diabetes, urinary tract infections, sexually transmitted diseases, RUBs and pregnancy outcomes that are statistically significant association. Conclusions: The result periodontal disease and preterm are not statistically correlated. Pregnant women with gestational periodontal was 31%. This finding confirms pregnant terms hormones change will led to cause specific of periodontal germs. The gestational periodontal of prevalence rate high that is pregnant women health of important issue. Health authority has to enhance oral health care advocates for pregnant women, and treatment and prevention of periodontal disease in pregnant women on the policy norms. The disease history of pregnant women should strengthen the control and treatment of the disease and improve quality of care for pregnant women, reduce the incidence of adverse outcome of pregnancy. Key Word: periodontitis, preterm birth, Resource Utilization Bands Ching-Wen Chien Song-Kong Huang 錢慶文 黃松共 2014 學位論文 ; thesis 75 zh-TW
