The purpose of this study was to evaluate the association between vertical positions of anterior teeth and smile esthetics assessed by Taiwanese dentists and dental students. A standard composite smile was constructed from a female’s intraoral and extraoral photographs. Vertical positions of anterior teeth were modified digitally in 1 mm increments from 5 mm upper lip coverage of the incisors to 5mm gingival exposure with total 11 images. Using a visual analog scale, 40 Taiwanese dentists and 82 dental students rated the attractiveness of 11 smiles with altered gingival displays. The results showed that the smiles with upper lip covering the anterior teeth 3 mm to gingiva exposed 1 mm were considered to be more esthetic. The male participants assigned higher scores to the smiles with upper lip coverage of the anterior teeth 2~4 mm than the corresponding females. Besides, the male dentists assigned higher scores compared to the male students for the smiles with upper lip coverage of the anterior teeth 4~5 mm. There was no difference between the scores assigned by the female dentists and the female students. The conclusion of this study: the smiles with upper lip covering the anterior teeth 3 mm to gingiva exposed 1 mm are considered to be more esthetic. The male have more acceptance to the lower smile line than the female. The dental professional training may affect the assessment of smile esthetic by the dentists.
