The Health Promotion Administration, Ministry of Health and Welfare, has aggressively invested in breast cancer prevention measures in hope to achieve the goal of early detection and early treatment. Due to lack of popularity of the mammography facilities at the hospitals in Taiwan, and mostly concentrating in the municipal areas, this has led to the lower rate of mammography screening test of the women. The sample hospital of the study has been using the mammography van for the medical facility lacking areas, to increase the conveniences of women receiving mammography screening test, and to assist the government to fulfil the social medical care activities. This research is to understand the difference and influence of the quality index of the sample hospital before and after using the mammography van, in order to achieve the goal of building up the quality control index of the social mammography screening test, to assist the assessment mechanism of the medical institutes, and to provide the management guides for supervision, assessment and decision making. After using the mammography van, the average monthly screening cases of the sample hospital have increased 9.8 times (before it was 104/month, after it was 1017/months.) The digital mammography van has brought the conveniences of screening test, and the increased availability of the medical services, so that the women of the medical lacking areas can also enjoy the welfare of the breast cancer prevention. The positive rate of breast cancer was 7.6% before, and 14% after using the mammography van. The follow-up rate for the positive cases was 88% before, and 52.8% after using the mammography van. This was probably due to longer distance to the hospital, traffic inconveniences, or personal reason refusing further biopsy examination. However, through the help of the nurses’ follow-up, the treatment rates for the diagnosed patients, whether before or after using the mammography van, were both 100%. My conclusion was that the hospital management unit should build up the quality control index and set up the goal value of the breast cancer screening test, review the abnormal indexes on regular basis, and proceed the cause analysis and improvement measures. The Administration should emphasize on advertising the benefits of breast cancer screening test, build up a complete transferring network, provide a convenient follow-up operating system, set up the rules for breast cancer screening test, and actively supervise the abnormal situations.
