description 碩士 === 美和科技大學 === 健康照護研究所 === 102 === Background and objectives The medical and healthcare sectors of Taiwan have been no stranger to the concept of “hospice nursing” for many years. Personnel involved in medical services and healthcare are familiar with this concept and how it works. The primary purpose of advocating for “hospice nursing” is to: “provide a certain quality life for the victims of cancer in the final stages, allow them to pass on with dignity, and pacify both the survived and the deceased”. This study aims at 4 objectives: (1) To find out about the knowledge, attitudes, and receptiveness that the families of patients under long-term hemodialysis treatment possess about “hospice palliative nursing” ; (2) To find out what factors the knowledge, attitudes, and receptiveness of “hospice palliative nursing” of the families of patients under long-term hemodialysis treatment should include; (3) To explore which demographic variables will affect the interviewees’ knowledge, attitude, and receptiveness of “hospice nursing”; and (4) To explore whether the attitude and receptiveness of the families of patients under long-term hemodialysis treatment to “hospice nursing” would be affected by more knowledge. Method and materials In this study, the cross-sectional design of survey is applied using a “questionnaire survey with anonymous respondents” completed by the families of the outpatients under long-term hemodialysis treatment. The survey covered the period of one month and a half, from April 1 to May 15, 2014, including interviews through questionnaires and the compilation of the data collected from questionnaires for analysis. Descriptive statistics (mean, percentage, frequency distribution, standard deviation) and inferential statistics (analysis of confidence level, analysis of validity, t-test, One-way ANOVA, straight-line regression analysis and path analysis) were adopted for verifying the hypotheses. Research findings In this study, there were 347 valid respondents (at a return rate of 86.8%) from the questionnaire survey. In general, the study of the samples in the areas of “medical and healthcare concepts and the relevant laws and regulations”, “frequently asked questions”, “identity of the idea”, “healthcare choice and implementation aspirations”, and “related measures” showed agreement with the advocacy for “hospice palliative nursing” by the sample hospital. The results of One-way ANOVA indicated that “the families of the patients under long-term hemodialysis treatment” were affected by their “level of education”, “occupation”, and “experience with caring for patients at the terminal stage of illness” in the samples collected. The results of the straight-line regression analysis indicated that:The demographic variables of the respondents showed that: level of education, marriage, occupation, religious belief, source of information on palliative care, high blood lipids, repetitive hospitalization, duration of dialysis, family status, knowledge about hospice nursing, contact with patients under hospice nursing, and identification with the concept of medical treatment, medical treatment choice, and implementation aspirations. The result of path analysis indicated that: “frequently asked questions” will affect “identity with the concept of medical treatment”, and “the choice of medical treatment and implementation aspirations”; and “identity with the concept of medical treatment” will affect “the choice of medical treatment and implementation aspirations”, and “related measures of hospice nursing”; and “the choice of medical treatment and implementation aspirations” will affect “related measures of hospice nursing”. Conclusions and recommendations From the questionnaire designed for the survey in this study, we could discover and explore the knowledge, attitudes, and receptiveness of the families to the patients under long-term hemodialysis treatment to “hospice nursing”. Of all of the demographic variables, only “level of education”, “occupation”, and “experience with caring for patients at the terminal stage of illness” affected the attitudes of the interviewees to the 5 dimensions of analysis. The research findings also indicated that all interviewees tended to “agree” with “hospice nursing” regardless of their different attitudes and degrees of understanding of “hospice nursing”. As such, the findings of this study lead to the recommendation that subsequent studies could proceed with a proper balance between “quality” and “quantity”. Their scope would expand to cover all of the families of the patients receiving hemodialysis treatment in the entire healthcare facility for the development of a model for the nursing of patients under hemodialysis treatment at the terminal stage of illness. Keywords: hospice nursing, patients at terminal stages of kidney disease, 8 major non-cancer diseases.
