Since 30 % of people having sleep disorders in Taiwan, sedative hypnotic drugs have become the most prescribed treatment by psychiatrist. In Taiwan, physician woks in a high pressure situation and some of them suffer from insomnia that requires treatment. However, there was no previous report concerning the different usage of hypnotics among different specialists of doctors. In this study, we aimed to investigate the use of hypnotic medications among different specialists and try to predict the risk factors of using hypnotic for insomnia doctors.
Methods:
This is a secondary data analysis study, subjects were recruited from registered of the “National Health Insurance Research Database” (NHIRD). The database belongs to a series of capital profile medical institutions, medical personnel basic profile, specialist certificate Master, outpatient prescription files which cover the period from 1997 to 201. All the analysis were carried out to describe the distribution of each variables by the SAS software package, version 9.2 (SAS Institute Inc., Cary, NCWith SAS). Chi-square analyses were computed to determine if there were any significant differences in demographic or characters in different fields. Logistic regression analyses were used to predict usage of hypnotics.
Results:
There were 644 specialists recruited in this study. Three hundred and ninety of them were prescribed with benzodiazepines class of hypnotics (BZD) and non- benzodiazepines (NBZD). In multivariable analysis, use of BZD and NBZD were statistically significant difference between groups of physicians and non-physician (OR=1.91, p=0.04), surgeon and non-surgeon (OR=2.45, p=0.00), and emergency doctor vs non-emergent doctor (OR=5.94, p=0.01). In the use of BZD, groups between physician vs non-physician, and surgeon vs non surgeon were statistical significance (OR=1.94, p=0.04; and OR=2.15, p=0.02, respectively). There was statistical significant difference between emergent and non-emergent physician of NBZD use (OR=8.28, p=0.00).
Conclusion:
Physicians, surgeon, and emergent doctor had a high tendency to use hypnotic medications. High pressure work environment maybe the possible cause. It is more obvious in emergent doctor. We suggest this useful information should be taken into consideration when the new medical policy is going to be drawn up by the government.
