Patients with CKD have higher incidence of cardiovascular disease, medical cost and mortality than those without CKD. In Taiwan, the incidence and prevalence of CKD are increased because of National Health Insurance policy, medical advance, increased elderly population and so on. It has been well-known that black race, dyslipidemia, hypertension, proteinuria, obesity, anemia and cardiovascular disease are risk factors of progression of CKD. Metabolic syndrome is an initiating factor of metabolic syndrome. However, it is unclear whether it is perpetuating factor for CKD. We conduct a retrospective cohort study to find perpetuating factors of CKD, including metabolic syndrome. Four hundred eighty five patients with CKD from Taoyuan General Hospital are divided into 4 groups and assessed. It is demonstrated that old age, decreased renal function at baseline, hypertension, dyslipidemia, proteinuria, anemia, hypoalbuminemia, hyperglycemia(diabetes mellitus) and metabolic syndrome are perpetuating factors of CKD. Patients with family history of hypertension, diabetes mellitus and gouty arthritis are prone to renal function deterioration. Although body mass index alone is not associated with progression of CKD, it is one component of metabolic syndrome. Based on the study, we should control hypertension, hyperlipidemia, hyperglycemia, proteinuria, anemia, malnutrition, hyperuricemia and metabolic syndrome to slow renal function progression.
