It is estimated that the ratio of the elderly population to the total will exceed 20 % by 2026. With the increasing elderly population, the delivery of needed nursing and caring services becomes a significant issue. In Tainan City, Longci District has the lowest population but the highest percentage of the elderly population. In Hsin-Feng area, a facility has been established by Family Service Centers with two care-managing professionals, who were responsible for long-term care assessment and transferring services in eight districts, including Longci, Guanmiao, Gueiren, etc. However, in the implementation there were difficulties, including how to conduct a census in such a vast area and locate the citizens who needed the health care. This study centers on four cases of high need subjects and describes, respectively, thoroughly and deeply their personal background, family and care process, then proceeding to a discussion where the information is integrated. After the discussion of the ratification of the issues mentioned above, it is suggested that the public sector should assist Longci District in establishing a localized facility which can provide the long-term and comprehensive services, catering to the need of the local citizens. In doing so, the concept that people take care of the people in their neighborhood is put into action and the goal to establish long-term care service model is achieved, thereby providing citizens in Longci District with appropriate long-terms service which is approximate, affordable, available and acceptable. Five essential elements which are as follows: professional competence, working experience, cultural background, possession of multi skills and personality.
