Compared to the general surgery, it combines radiation hazards and contrast media hazards, and the injury by human mistake An efficient, efficient management and control tool that can reduce operational risk and to improve patient’s safety. Objective: The purpose of this study is tying to use the concept of risk factors, production implementation complies with ISBAR Introduction, Situation, Background, Assessment, Recommendation process, by SOP checklist, and to evaluate their effectiveness of patient’s safety. Methods: ISBAR namely the use. From February to April in 2013, under a regional teaching hospital’s, vascular photography help to set the SOP. Furthermore to investigate three most commonly angiography operations, as focus group of 100 patient data in all, to and compare 2012 and 2013 two-year error rate. Results: By ISBAR established the SOP, and transfer to checklist. Most of samples(85%) were satisfied on checklist, the average score is 41.5.Before to implementing the checklist, overall error rate was 31.1%, including failure to check before go to prepare (24.5%), did not sign checking consent or the condition explaining of patients(4.7%), did not exam blood values 1.9%.After implementation of checklist, the overall error rate is reduced to 4.3%, including not signed to check consent or explain the disease, not exam the blood values, they were reduced to 0%.While failure to check before going prepare item was down to the 4.3%.After and before implementing checklist, the improvement rate was 86.2%. Conclusion: According ISBAR principle to design the SOP for a checklist not only has effects on nursing shift but also works on the operations of angiography.
