The knowledge and attitudes of nursing supervisors in elderly welfare institutions about residents’ signing Do Not Resuscitate (DNR) orders would affect the quality of caring end-of-life residents. The purpose of study is to offer reference to promote palliative care policy in elderly welfare institutions. Methods: The subjects participating in the study were 1054 elderly welfare institutions registered in the ministry of the interior in Taiwan which were recruited by systematic sampling according to the amount of each county/city. 345 questionnaires were sent out while 105 (46.8%) were returned. The software, SPSS 18.0, was applied for the following statistical analysis, including descriptive statistics (number assignment, mean, SD, %) and inferential statistics (t-test, one-way ANOVA, Scheffe’s posteriori comparisons, Pearson product correlation). Results: (1) The average score for cognition on Not Resuscitate was 12.45 (16 in total, SD = 1.850), and the correct response rate was 77.81%; (2) The average score for attitudes toward palliative care was 50.5 points (60 in total, SD = 4.48); (3) The knowledge and attitudes of nursing supervisors in elderly welfare institutions about residents’ signing Do Not Resuscitate (DNR) orders are significant related. The higher score of cognition on Not Resuscitate is, the more positive attitudes toward palliative care are. Conclusion: The attitude tendency of nursing supervisors in elder welfare institutions about Not Resuscitate is good in Taiwan while the palliative class receiving rate is not ideal and the cognition toward palliative care is comparatively low. The competent authority should improve the cognition of nurse supervisors in elderly welfare institutions on palliative care so that the quality of caring end-of-life residents would be enhanced.
