Description
Summary:碩士 === 長庚大學 === 中醫學系傳統中醫學 === 101 === The prevalence of obstructive sleep apnea syndrome (OSAS) is closely associated with metabolic syndrome, diabetes and cardiovascular diseases including hypertension, stroke and ischemic heart disease in which caused high-mortality in epidemiology. Continuous positive airway pressure (CPAP) has reported to treat OSAS efficiently, yet it could not be accepted for long term use by most of the patients. According to tranditional Chinese medicine(TCM), pattern identification plays a pivotal theoretical basis in amelioration of weak-physique as well as morbid- reduction. A“snoring pattern”is the major pattern identification in OSAS in TCM clinically. In addition, recent studies from Mainland China had proved that symptoms of phlegm-damp, qi-vacuity, damp-heat and blood stasis had also been addressed as the pattern identification of OSAS. However, the related guidelines for TCM are not completely identified yet. The aims of current study are firstly to categorize the various subtypes of pattern type of OSAS in accordance with pattern identification depends on the previous references and dissertations. Secondly, different symptoms and occurred frequency were represented from above subtypes of pattern type by statistics, subsequently to explore the trend index of variable value from pattern type's database using Contingency Table of Cross Classified methology . Finally, to analyze characteristics of tongue image and pulse manifestation according as tongue and pulse examination records from OSAS patients in Taoyuan Chang Gung Memorial Hospital . Furthermore, we also investigated whether divergence of characteristics after with or without CPAP challenge. Our results have been illustrated that the highest-relationship between OSAS development and “Qi” disturbance; meanwhile, “Organs” were focused on lung, spleen and kidney. On the other hand, formation of vacuity, phlegm and stasis occupied the most important role in pathomechanism of diseases. According to clinical records from Sleep Center of Taoyuan Chang Gung Memorial Hospital, tongue bearing dental impressions and enlarged tongue had been alleviated in patients with OSAS in early stage of CPAP therapy, simultaneously pale red tongue and cracked fur might also been ameliorated after long-term OSAS treatment, but improvement of red prickly tongue was not altered significantly. Before CPAP treatment, pulse manifestation was represented as pulses of deep, weak, low and unstringlike and/or pulses of deep, weak, small, low and unstringlike, neverthless the alterations of pulse had not obvious occurred through CPAP therapy.
