Physical restraint use is a quality indicator in insititutional long-term care. Purpose: The purpose of this study was explored the relationships among the knowledge and attitude of physical restraints, work-related stress and empowerment of nurse aides, and the physical restrained residents and its related factors. Method: This study was a cross sectional study. A total of 130 nurse aides and 727 residents from four participate long-term care facilities in the study, from January to November 2012. Demographic characteristics and self-administrated questionnaires including,「Knowledge of Physical Restraint Scale」,「Attitude of Physical Restraint Scale」,「Work-related Stress Scale」,「Empowerment Scale」 and 「Physical Restrained Residents Checklist」were also used. The statistics including Chi-square, independent t-test, analysis of variance (ANOVA), Pearson’s correlation, and Logistic regression were used to analyze the data. Results: The results showed that nurse aides were(1)average scores of knowledge of physical restraints were 62.46%;(2)attitudes of physical restraints ranged from “agree” to “not agree”;(3)“moderate” work-related stress ,especially “worry about residents' condition deteriorated was the highest level of stress;(4)perceived empowerment level ranged from “some” to “a lot of”, especially “there is reward of recreation in the work” dimension was the lowest score. There were significantly positive correlation among nurse aides knowledge, attitudes of physical restraints and empowerment, as well as negative correlation among physical restraints and work-related stress. The prevalence of physical restraint were 22.56%;The ADL scores and the tube of number of residents were predictors of physical restraints, and the odds ratio(OR) were .967 and 1.99. The knowledge and attitudes of physical restraints, work related stress and perceived empowerment level of nurse aides were not significantly associated with the physical restraints. Conclusion and clinical implication: Further interventions focused on alternative management of physical restraints will be needed to facilitate nurse aides' positive attitudes of managing residents behaviors, provide positive reward and feedback to nurse aides to empower their decision making and reduce physical restraint in long-term care facilities.
