In Taiwan, The Department of Health is actively promoting EMR exchange, which is still in a germination stage. Some literatures indicate that the intention of EMR exchange is cautious in domestic hospitals, because IT investment is more expensive for Taiwan's hospital in the background of National Health Insurance (NHI). It is the reason of the lack of information technology and cost. Therefore, this study expects to know whether the hospitals adopt Cloud services to reduce the IT and cost obstacle about EMR exchange. The study is based on Technology - Organization - Environment (TOE) framework and the construct of hospital performance to develop an integrated model. The study chooses the outstanding performance hospitals in 2010 implementing EMR exchange for the sample published by Department of Health, Executive Yuan. A total of 231 questionnaires are mailed to the information technology staffs of medical centers, regional hospitals, and local hospitals. 83 questionnaires are returned and the response rate is 36%. The study use SPSS 20.0 and SmartPLS statistical software for data analysis. The results indicate that Cloud services, formalization, top managers support, government policy, and competitive pressure have significantly effect on the intention of EMR exchange. Finally, EMR exchange has much effect on medical quality and operational performance.
