This study aims to integrate two systems, Taiwan Patient Safety System (TPR) and Taiwan Healthcare Indicator Series (THIS), to establish a medication incident monitoring system. The medication incident monitoring system combines the advantages of reporting with monitoring continuously. Different kinds of control charts are surveyed in order to find the appropriate one for medication incident. The shewhart control chart is the most popular choice for monitoring medication incident. To consider the severity factor, the researcher conducts a multilevel shewhart control chart. For detecting small shifts, the researcher designed a binomial exponential weighted moving average control chart. For both severity and small shifts detecting, multilevel exponential weighted moving average control chart is the appropriate tool. In addition, the researcher combines Taguchi loss function with multilevel exponential weighted moving average control chart to minimize quality loss. The researcher developed an evaluation system to find the appropriate parameters to detect minimum loss.
