BACKGROUND
Chronic obstructive pulmonary disease (COPD), a common comorbidity for long-term hemodialysis patients, burden the lung in the process of hemodialysis with water heavily clinical care mainly focus on hemodialysis but ignore the seriousness of chronic obstructive pulmonary disease patients. Therefore, weconducted this study to figure out the mortality and the impact of health care expenditure among hemodialysis patients with chronic obstructive pulmonary disease.
METHOD
In this study, 2003-2006 National Health Insurance Research Database are used to conduct a retrospective case-control study, werun chi-square test, independent sample t-test and generalized liner model(GLM) to test the effects of health care expenditure for hemodialysis patients with chronic obstructive pulmonary disease; to test the effects of Kaplan-Meier method and Cox's proportional hazards the model for inferential statistic to confirm risk of death.
RESULT
435 cases and 1,740 patients, as control group, we included (age, gender and health status of 1:4), the results show than long-term hemodialysis patients with chronic obstructive pulmonary disease complications will increase the risk of death, but no evidence to suggest that a significant influence on total health care and expenditure. By age and other comorbidities such as diabetes, cancer, dysrhythmia, liver disease, cerebral vascular accident/ transient ischemic attack and gastrointestinal bleeding diseases show significant impact on total health expenditure.
CONCLUSION
This study found that in the mortality was significantly higherin hemodialysis with chronic obstructive pulmonary disease, and complications of chronic obstructive pulmonary disease.The urgent need for further research to explore for hemodialysis patients with chronic obstructive pulmonary disease and seeking a better quality of health care.
