In Taiwan, approximately 90% of adults have been infected, and around 15-20% of adults are chronically infected with HBV. It is therefore a common thing to find patients who show a positive hepatitis B surface antigen in their blood serum. Since hepatitis B is such a common and severe disease in Taiwan and also many other countries of the world, the treatment of it is an important issue nowadays. There are two kinds of drugs available for the treatment of hepatitis B in western medicine such as oral antiviral drugs and interferon. Both of them can reduce viral replication, the inflammation of liver tissue, the cirrhosis and the incidence of liver cancer. However, they cannot remove the virus completely and the patients also need to take them for a long time. That is why nowadays many patients even in western countries consider a treatment with Chinese herbal medicine, alternative therapies or natural products. Although their efficacy were reported in many cases, most of these reports were lack of a well planned study design or some of them were even just report the individual cases. In a modern culture, where medical advances could help to increase the survival rates, life quality is now an important target to work on. According to Traditional Chinese Medicine treatment strategy, we designed Fuzhenh prescription and Zhong-Hua-Fu-Qi-Yin decoction (ZD) to reduce the viral load by enhancing the health status on abnormal liver function of HBV carriers. The purpose of this study was to evaluate whether ZD could improve the liver function, viral load and life quality of hepatitis B patients. This trial was conducted on February 2011 to December 2011 in Linsen (TCM) branch, Taipei City Hospital. Fifty seven of eighty six HBV carriers, who fit to our inclusion and exclusion criteria, completed this study. The participants were randomly divided into two groups: the ZD Group (n=29), who took the concentrated Zhong-Hua-Fu-Qi-Yin Decoction and the control Group (n=28) who received a 16 times diluted one. The participants were asked to take 1 pack of the decoction after every day´s dinner for 6 weeks. After six weeks treatment, the ZD group showed a significant reduction of the liver function and enhancement of life quality on physic score, although the serum viral load did not reach statistical difference. Although, the HBV DNA level did not improve significantly, a decreasing trend after six weeks could be recognized in the ZD group. However, in the control group, no significant change could be shown after six weeks. We only found a significant reduction of the glutamate pyruvate transaminase (GPT) in both groups which showed a reduction of 25.2% in the ZD group and 0.74% one in the control grou. There was no major adverse effect in two groups. The present study showed that the ZD significantly improves the liver function on abnormal liver function of HBV carriers.
