The derivatization of buprenorphine and norbuprenorphine had been reported to undergo chemical rearrangement and result in poor reproducibulity. This study was designed to establish a reliable LC-MS/MS method for the analysis of methadone, buprenorphine, opiates and their metabolites in urine specimens by high-resolution liquid chromatography tandem mass spectrometry in human urine was developed ; Chromatography separation was achieved in 5.2 min and quantification was performed by multiple reaction monitoring. Extraction efficiencies were more than 68 %. Intra-day and inter-day precision was less than 12.1% and accuracy was between -9.8% and 13.7%. Although ion suppression was detected, deuterated internal standards compensated for these effects. Carryover was minimal, less than 0.20%. All analytes were stable at room temperature for 16h, 4 ℃ for 72h, and after three freeze-thaw cycles. The assay also fulfilled compound identification criteria suggested by the European Union. The method was applied to 62 urine samples collected from patients following methadone or buprenorphine maintenance therapy. More than half of urine samples are opiates-positive. This method provides a fast, reliable and simultaneous quantification of opiates, maintenance drugs , and their metabolites in urine sample.
