The quasi-experimental design and volunteer sampling were utilized. Subjects were recruited from a community center for the elderly with 52 samples in the experimental group and 30 samples in the control group. Healthy promoting lifestyle surveyed by questionnaire, and objective health status in blood pressure, blood sugar, lipids, BMI and Waist- hip ratio of the 2 groups were collected before and after the program. The average age of the subjects was 74.5 years old (SD=3.963). Majority of them were women, widowed, illiterate, living with families, retired, and with NT $5,000 living expenses per month. 83.3% participants had exercise habits, but suffering from chronic illnesses, especially hypertension. There was no significant difference between two groups for their demographic factors on baseline data. Effects of this program were shown: (1) results of the post test were indicated that scale of the health promoting lifestyle were better in experimental group. (2) both systolic and diastolic blood pressure were significant better in experimental group. (3) in experimental group, the results of the paired t-tests indicated the significant improvement in the dimensions of health responsibility, exercise, nutrition, self-actualization, interpersonal relation and stress management. (4) educational level, gender, living condition, and exercise habits were significant factors related to effect of the health promoting lifestyle of the elderly. According to the results of the study, health management program with a small-group health education and follow-up vital sign health management did improve some health indicators and promote healthy lifestyle for the community-dwelling elders. Some suggestions for the community centers and government are provided.
