The structure of diseases has turned from acute infectious diseases in early times to chronic diseases currently. Different degrees of disability caused by chronic diseases are the major reasons for modern people to receive medical services. About 56% of the elderly above 65 years old suffer from chronic diseases and 5.43% of them cannot live independently and require the care by others. Thus, because of transformation of diseases and aging population, the demand for “care in life, with the support of medical care” of long-term care will considerably increase. With the progress of medicine and health, life quality in Taiwan increases significantly. Since the average life-spans of males and females increases, the issues of aging in the society and the related social meanings should be concerned. With rapid aging of population, since the government cannot satisfy all needs and shoulder the full responsibility of long-term care, the sharing of responsibilities between individual and the public is a more practical approach. According to the trends of long-term care reform in different countries and social welfare development in Taiwan, demand for long-term care is increasing and there is requirement for upgrading and diverse nursing quality. Thus, future long-term care supply should be based on the responsibility and cooperation between the government and private participation. The development of long-term care resources will be privatized. As to services, private profit and non-profit organizations are encouraged to participate in development of service resources. Upon the resources and force of individuals, the public, private and non-profit organizations, and the operation of the elderly business, the diversity of services and options for the people will be enhanced. According to the “Promotion of Private Participation in Infrastructure Projects” and multiobjective use, this study analyzes the current nursing institutions in Tainan City, conducts financial analysis by simulating private participation in construction of nursing centers, assesses construction, selection of bases, legal propriety, market, finance and environment of private participation to find out how the elderly nursing institutions in Tainan City match social welfare and maintain investment benefits of business, in order to provide more complete care service.
