The “Leisure Sport Cognition, Exercise Self-Efficacy and Physical and Mental Health for Elementary School Teachers Questionnaire” adopted a stratified cluster sampling method, which sampled elementary school teachers in Taichung City to conduct the research. Subjects were 800 elementary school teachers in Taichung City. The effective sample was 729 samples. An effectiveness rate of 91.13% was reached. The statistical analyses were analyzed by SPSS for Windows 12.0. After the data was analyzed by descriptive statistics, one-way ANOVA, Pearson’s product moment correlation analysis, and stepwise regression analysis. The major findings wrer as follows: 1.The personal background were significant differences to leisure sport cognition, yet the marriage conditions and the position were not. 2.There were significant positive relationships of the dimensions of leisure sport cognition and the dimensions of exercise self-efficacy. 3.There were significant positive relationships of the dimensions of leisure sport cognition and physical and mental health. 4.The abundant life and self-approval levels of leisure sport cognition could predict exercise self-efficacy. Social levels could predict whole exercise self-efficacy, external environment and self-feeling involved. The relaxation of body and mind could predict social support levels. 5.The social support levels of exercise self-efficacy could predict life goals, mental perception levels of physical and mental health. The whole exercise self-efficacy could predict emotional responses, body perception, social interaction and whole physical and mental health.
