A total of 3 bridge construction projects in the central Taiwan region were selected to provide typical field facility safety data during design, installation, and dismantle stages of the span by span bridge construction techniques. In the study, the practical project data were collected, analyzed, and discussed to reveal that facility safety failures were contributed to lack of drawing interpretation and specification of facility safety operation, and inadequate implementation of the required safety reinforcement conducts during each standard operation procedure of installation and dismantle of span by span construction of the bridges. Results of the study indicated that factors lead to facility safety failures with injury and casualty also include lack of daily safety brief and education to the labors, and ineffective field safety supervision throughout the entire span by span bridge construction process related to: (1) records of deformation and dislocation of bridge span forms in the events of significant natural hazards such as storm, earthquake, and typhone, (2) inadequate or insufficient field supervision to avoid incorrect sequence, timing, and traffic interference during concrete pouring leading to instability sometimes collapse of the bridge span. In the research , the causes of safety facility failures for the selected practical bridge projects were identified and studied to develop necessary improvement tactics to minimize the risk of injury and casualty resulted from facility safety failures during the similar cast in-situ span by span bridge constructions in the future.
