Understanding the reasoning and decision-making process used by medical professionals to resolve the challenges is important. This study aims to investigate the differences in ethical judgment between nursing students, nurses and nursing administrators. The results of this study can help health care providers to improve the training outcome of nursing professionalism. A semi-structured interview was employed in this study. A total of 60 interviewees, included nursing students (20), nurses (20) and nursing administrators (20), were interviewed. According to interview results, the reasoning used by interviewees were categorized as intro-control, extro-control and mixed. The results suggest that the reasoning used by nursing professionals to solve the ethical challenge is characterized by (1) being more family oriented than patient autonomy; (2) too much emphasis on peer relationship; and (3) preference for external control to internal control. While everyday challenges to professionalism are commonly encountered by trainees and practicing nurses, many practitioners received few training in this area and therefore were unable to provide a consensus answer to these challenges. It is important to establish apprenticeship and on-job training programs to internalized professionalism ethics by using problem-based learning and narrative writing methods so that nursing professionals become more sensitive to ethical challenges faced in everyday work environment.
