碩士 === 國立陽明大學 === 醫務管理研究所 === 99 === Purpose: Title of thesis is the Utilization of Tourette syndrome and the influencing factors in Taiwan. This study has two major purpose: First, to understand continuity of care how to impact Tourette’s patient health status. Second, to understand the utilization of Tourette syndrome in Taiwan. Materials and Methods: This thesis is cross-sectional study. We collect declared information from bureau of national health insurance of patient in 2006 which is provided by National Health Research Institutes, including underwriting file, CD, DD, OO and DO files. We select all Tourette syndrome patients in 2006, then use COCI (continuity of care index) to calculated their continuity of care. To analyze sex, age and psychiatric complications factors how to impact Tourette’s patient medical utilization. On the other hand, this study is focus on the patient’s disease of burden. The major findings of this study are summarized as follows: Tourette syndrome patients who have higher continuity of care will reduce patient’s disease of burden and their medical utilization. Patient’s age increase will rise their disease of burden. About Tourette syndrome patients whether they have psychiatric complications. If patient who has psychiatric complications, then their disease of burden and utilization of medical resources will rise. Recommendations: 1. To provide Tourette syndrome patients integrated care model and health care network. 2. Using capitation payment for the management of patients with Tourette syndrome. 3. To provide Tourette syndrome patients professional medical care institutions. 4. We suggested future researchers may have long-term research. 5. To analyze the reason of Tourette syndrome patients have lower continuity of care in Taiwan. 6. We suggested future researchers can use other indicators to calculated patient’s continuity of care. 7. We suggested future researchers can consider the characteristics of hospital. 8. To analyze the relation between continuity of care, sex, age and psychiatric complications. 9. To analyze the relation between disease of burden and health care utilization.
