A survey was mailed to 360 professional working in related teams from 72 centers, and a qualitative data was collected as a focus group interview of 10 written report users. The result indicates the comprehensive written report was limited to the evaluation model and approaches. Mostly professional driven the evaluating process but not put the parents’ participation in concern and few ommunication in between while composing the report. The treatment program afterwards is hardly follow-up by the original evaluation team. In professional’s view, better written form, and more support resources such as equipments are needed. The evaluation procedure, written report delivery, the time consuming issue, and the information needs to be simplified and clear are required based on the users’ response.
