Summary: | 碩士 === 國立東華大學 === 電機工程學系 === 99 === Although previous studies have shown the successful use of pressure-induced reactive hyperemia as an assessment tool of endothelial function, the sensitivity remains questionable. This study aims at investigating the feasibility and sensitivity of a novel multiscale entropy index (MEI) in detecting subtle endothelial dysfunction in healthy and diabetic subjects. Between July 2009 and October 2010, totally 97 subjects were recruited from Hualien Hospital, Taiwan, and categorized into Blood Sugar AC < 90 mg/dl group (Group 1), 91mg/dl <Blood Sugar AC < 125mg/dl (Group 2), Blood Sugar AC > 126mg/dl (Group 3). Basic anthropometric, hemodynamic, serum lipid profiles, and level of glycosylated hemoglobulin were recorded. Arterial pulse wave signals were acquired from the wrist with air pressure sensing system (APSS), followed by MEI between for Amplitude (AMP) and Pulse-Pulse Interval (PPI), dilatation index (DI) analyses. The results showed that AMP-MEI succeeded in detecting significant differences among the three groups, whereas there was no notable difference in DIs and PPI-MEI between Group 1 and Group 2. In conclusion, a reduction in multi-scale entropy reflected age, Blood Sugar AC and diabetes related vascular changes and may serve as a more sensitive indicator of subtle endothelial dysfunction compared with DI in the setting of diabetes.
